Just started Mounjaro or Wegovy? You might be wondering whether you actually need to exercise, or whether the medicine is doing enough on its own.
The honest answer is that you can, and should, stay active alongside either treatment. It won't stop the medicine working, and exercise plays a bigger part in your results than most people realise.
This guide covers how much exercise you need, which type matters most, whether it's safe to exercise around dose increases, and what tends to happen if you skip it. Still deciding between treatments? Our oral semaglutide guide covers how Wegovy's tablet form compares with the injections discussed here.
The medicine helps you eat less. Exercise helps decide whether the weight you lose comes from fat or muscle.
Can you exercise on Mounjaro or Wegovy?
Yes. Exercise is safe alongside both treatments and doesn't interfere with how they work. Mounjaro and Wegovy act on your appetite and digestion, not directly on your muscles or heart, so the medicine and exercise simply work side by side.
Both treatments are licensed for use alongside a reduced-calorie diet and more physical activity, not as a stand-alone fix.1,2
The one thing worth planning around is how you feel in the first few weeks. Nausea and tiredness are very common while your body adjusts to Wegovy, and tiredness is a common side effect of Mounjaro.1,2
Neither is a reason to avoid exercise altogether. It's usually easier to lower the intensity for a week or two than to stop being active completely.
Will exercise make Mounjaro or Wegovy side effects worse?
For most people, no. Many find that gentle activity, like walking, helps their digestion and eases mild nausea rather than making it worse.
The exception is the few days after a dose increase. That's when digestive side effects tend to be at their strongest with both treatments, as your body adjusts to the higher dose.1,2
Just moved up a dose? Swap your toughest session that week for a walk, then build back up.
There's no need to stop exercising around a dose change. Lowering the intensity for a few days, staying well hydrated and paying attention to how you feel is usually enough.
Nausea or tiredness stopping you exercising for more than a week or two? Mention it to your prescriber, as it may be worth reviewing how quickly your dose is going up.
How soon after starting Mounjaro or Wegovy should you start exercising?
There's no need to wait. You can start or carry on with your usual exercise routine from your very first dose, as long as you feel well enough.
Some people prefer to ease into a new routine over the first week or two, while nausea and appetite changes settle. That's a reasonable approach, but there's no clinical reason to delay altogether.
If you weren't very active before treatment, the first few weeks are actually a good time to start building the habit. Your appetite is often lower early on, so you may have less energy for intense exercise. But lighter activity, like walking, is usually well tolerated from day one.
Is it safe to exercise on an empty stomach on Mounjaro or Wegovy?
Both medicines reduce your appetite, so it's common to go longer between meals than you're used to. Exercising on an empty stomach is generally fine for light to moderate activity, like walking.
Be more careful with intense sessions, though. Especially if you also take a diabetes medicine such as insulin or a sulfonylurea, since the combination can raise your risk of low blood sugar.1,2
On insulin or a sulfonylurea too? Keep a sugary snack with you during exercise in case your blood sugar drops.
Feel light-headed, shaky or unusually tired when you exercise on an empty stomach? A small snack with some carbohydrate and protein, such as a banana with a spoonful of nut butter, 30 to 60 minutes before a harder session is a sensible precaution.
Do you need to exercise while taking Mounjaro or Wegovy?
Strictly speaking, you'll probably still lose weight without exercising, because both medicines work mainly by reducing how much you eat. What exercise adds is more control over what kind of weight you lose.
Fast weight loss from a calorie deficit costs some muscle as well as fat. In the SURMOUNT-1 trial of tirzepatide (Mounjaro's active ingredient), around a quarter of the weight lost was lean mass. That was similar to the placebo group, who lost weight through diet and lifestyle alone.3
That figure is an average across the whole trial, though, and it hides a lot of variation. People who stay very inactive during treatment tend to lose a bigger share of muscle than those who keep moving.
So exercise still matters, even though the headline number looks manageable. You can read more, including who's most at risk, in our guide on whether Mounjaro or Wegovy causes muscle loss.
Does exercise affect how well Mounjaro or Wegovy works?
Exercise doesn't change how semaglutide or tirzepatide work in your body. Both act on GLP-1 receptors (and, in Mounjaro's case, GIP receptors too), whatever your activity levels.1,2 What exercise changes is the result you get from that weight loss, not how the medicine works.
It's also worth knowing where the headline figures come from. That's around 15% of body weight for Wegovy's standard maintenance dose, and up to 22.5% for Mounjaro's highest dose.
Both come from trials where participants also had structured diet and lifestyle support as part of standard care.4,5 The medicines were never tested without that support. It's one reason real-world results can fall short of trial results when the lifestyle side is left out.
How much exercise do you need on Mounjaro or Wegovy?
The standard NHS guidance for adults applies here. There's no need to do more just because you're on treatment.6
- At least 150 minutes of moderate activity a week, or 75 minutes of vigorous activity, or a mix of both
- Strengthening activities that work the major muscle groups on at least 2 days a week
A simple way to judge intensity is the talk test. During moderate activity, like a brisk walk, you can hold a conversation but not sing. During vigorous activity, like running, you'll only manage a few words before you need to pause for breath.6
Does 150 minutes sound like a lot? It's fine to build up gradually. If you haven't exercised in a while, you don't need to hit that figure in week one.
Two short walks and one light strength session in your first week is a reasonable start. Then aim to build towards the full guidance over the following months.
What's the best type of exercise on Mounjaro or Wegovy?
Most of the benefit comes from combining two types of activity rather than relying on one:
- Resistance training - lifting weights, using resistance bands, or bodyweight exercises like squats and push-ups. This is what protects your muscle while you're in a calorie deficit, and it's the type of exercise people on GLP-1 medicines most often skip.
- Cardio - walking, cycling, swimming, or anything that raises your heart rate for a while. This supports your heart health and blood sugar control, and both Mounjaro and Wegovy have been shown to improve heart-related risk factors in trials that combined treatment with lifestyle support.4,5
If you can only manage one to begin with, resistance training generally has the bigger effect on your body composition. Both matter for your overall health, though.
A simple starting point for each:
- Resistance training examples - bodyweight squats, push-ups, resistance band rows, or a pair of light dumbbells for moves like bicep curls and shoulder presses. None of this needs a gym membership to begin with.
- Cardio examples - a brisk 20 to 30-minute walk, a short bike ride or a few lengths of a pool. Low-impact options are worth prioritising if you're carrying extra weight or have joint discomfort, since they're easier to keep up.
What if you can't exercise much yet?
Starting from a lower fitness level, carrying a lot of extra weight, or managing joint pain? The guidance above is something to build towards. It isn't a condition you need to meet before you start treatment.
Something is almost always better than nothing. A few minutes of standing and moving around during the day, seated resistance exercises or a short walk to the end of the road all count as a start.
Many people find that as their weight comes down, joint pain and breathlessness ease too. That makes it easier to build up activity over the following months, rather than forcing it from week one.
If a health condition makes you unsure what's safe, a GP or physiotherapist can help you find the right starting point.
How to tell if your exercise routine is working
The scales don't tell the full story, especially in the first few months. Your weight can stay the same or move slowly even while your body composition is genuinely improving. That's because muscle is denser than fat, so it takes up less space for the same weight.
Better things to track alongside the scales include:
- How your clothes fit, especially around your waist
- Whether everyday activities like stairs or carrying shopping feel easier
- Whether you can lift a little more, do a few more reps or walk a bit further than when you started
- How you feel day to day, including your energy levels and sleep
If several of these are improving even when the number on the scales isn't moving much, your routine is very likely doing its job.
What happens if you don't exercise on Mounjaro or Wegovy?
You'll most likely still lose weight, since the appetite-reducing effect of both medicines carries on whatever your activity levels. What tends to change is what that weight loss is made of, and how you feel day to day.
Without resistance training in particular, more of the weight you lose is likely to come from muscle rather than fat. Over time, that can mean feeling weaker, having less energy for daily tasks and finding it harder to keep your results if you reduce or stop treatment.

Frequently asked questions
Can you lose weight on Mounjaro without exercising?
Yes. Both medications reduce appetite enough to cause weight loss through diet alone. Without exercise,
particularly resistance training, a larger share of that weight loss is likely to come from muscle rather
than fat, which is why staying active is recommended rather than optional.
Do you have to exercise every day on Mounjaro or Wegovy?
No. NHS guidance recommends at least 150 minutes of moderate activity spread across the week, plus 2
strength sessions, not daily exercise. Spreading activity across 4 to 5 days with rest days in between is a
sustainable approach for most people.
What's the best exercise for weight loss on Mounjaro?
There isn't a single best exercise. Combining resistance training, such as bodyweight squats or light
weights, with regular cardio, such as walking or cycling, gives the best results by protecting muscle mass
while supporting heart health and blood sugar control.
Can you still exercise on Mounjaro if you have joint pain?
Yes, though low-impact options such as swimming, cycling, or walking are usually more comfortable than
high-impact activities like running. Joint pain often eases as weight loss progresses, making it easier to
build up to more varied exercise over time.
Should you exercise on Mounjaro if you feel nauseous?
Light activity such as walking is usually fine and can sometimes ease mild nausea. It's best to save
more intense sessions for when you feel better, particularly in the days following a dose increase, when
nausea tends to be at its strongest.
How much exercise do you need each week on Wegovy?
The same as general NHS guidance for all adults: at least 150 minutes of moderate activity, or 75
minutes of vigorous activity, plus muscle-strengthening exercise on at least 2 days a week. There's no
additional requirement specific to Wegovy.
